Apply for EOPS

You can apply for EOPS at any time (no deadlines).

This is the EOPS Application.  The EOPS application form is required of all students in the program.  Students who are new to our program are required to complete an EOPS application only once UNLESS the student discontinues with the program. Students may bring a completed application to EOPS by downloading and printing a copy below. Students may also come into the EOPS office for a copy. In addition, a CARE application may be downloaded below if a student is eligible to become part of the CARE program. If you have any questions, please call EOPS at 650.306.3300.

Am I eligible for EOPS?

To help students succeed, EOPS provides academic support services that are specifically designed for each student’s special needs. Students that are accepted and participate in EOPS must meet eligibility criteria which certify them as low income and educationally disadvantaged. In order to be accepted and to participate in EOPS, students must meet the following eligibility criteria:- Must be a California Resident
- Must be eligible for BOG A or Bog B * (Students eligible for BOG C are NOT eligible for EOPS)
- Must be enrolled or plan to be enrolled full-time (12 units or more)
- Cannot have earned more than 70 degree applicable units*
- Must have at least one educational disadvantage (tested into remedial English and Math; English Second Language (ESL); have not graduated from high school or earned a high school grade point average (GPA) of 2.49 or lower)

All potential EOPS students must meet with all eligibility requirements to determine final eligibility

Instructions

Individuals need to have Adobe Acrobat Reader (Adobe Reader) software to download the forms on this Website. Students may download the software for free from the Adobe Website (http://www.adobe.com/). Search for a button or link "Get Adobe Reader."

Copies of all forms may be obtained as well at the EOPS/CARE Office (Building 3, Room 117). For those with a completed BOG Fee Waiver Application, the EOPS application may be completed and submitted to the office immediately, or may be returned it later. Applicants are not required to have the BOG Fee Waiver Application completed to apply. To obtain the EOPS application,
1. Click on a link below to download a form.
2. Select "Open" to open the form. (You will need Adobe Reader. See paragraph above.)
3. Print the form. (Pull down the "File" menu and select "Print.")

How to Apply

1. Complete the BOG Fee Waiver Application from CC Financial Aid Office website.
2. Complete the EOPS Application.
3. Complete the EOPS/CARE Responsibility Contract.
4. Make a copy of the forms for your records.
5. Bring or mail the completed forms to EOPS.
