Class workshop Request Form


This page is for librarians to add, search, delete or modify records in the workshop database.
To add a record, fill in the form below, then click on the "Add" button at the end of the form. To search for records in the database, enter your search criteria in the form (you don't have to enter all the required fields), then click on the "Search" button at the end of the form to search the entire database. Leave the form blank to show all records. To modify or delete records, click on "Search/modify" button at the end of this page.

Click 'Search' to search for records in the database.

To search, modify, or delete records in the database, click 'Search/Modify.' Enter password and you will be linked to an "Administration Page" where you can enter criteria to search, modify, or delete records in the database.
