Tutor's Name:  _____________________________

Please review this contract with your supervisor.  Some of the items may not be appropriate for your position.  Your supervisor will cross out the appropriate items.

As a tutor I am responsible for the following:

 1) Maintaining the student's confidentiality.

 2) Complete on-line Tutor Training and/or attend regular weekly meetings on Mondays at 2 pm. You will be credited for this time.

 3) Completing an audio tutor/learner observation evaluation and/or an audio tutor/learner role-play observation.

 4) Being on time and prepared for all tutoring sessions.

 5) Notifying or telephoning my supervisor and my assigned students when I cannot keep a tutoring appointment at least 12 hours prior to my appointment. *Please be advised that being on time when you are scheduled to work is very important.  When you were hired, you gave us your availability and agreed to the posted schedule.  Students expect tutors to be here when they have signed up for an appointment or for drop in.  If you are late, either the students waiting don’t get tutored or another tutor has to stay later, therefore disrupting the flow of our services.  If you are going to be absent or late, please call 738-4144 prior to your tutoring time and let Linda Allen or your supervisor know.  Three tardies of more that 10 minutes will be cause for termination.

 6) Contacting all scheduled students who do not keep their tutoring appointments and completing the follow up form. (EOPS Tutors)
